Child Health Plan Plus

Child Health Plan Plus (CHP+) is a low-cost health and dental insurance for Colorado children and pregnant women. The program was created to help families who make too much money to qualify for Medicaid but not enough to afford private insurance.    

Depending on family size (people in your household that are related) and income, you may need to pay an enrollment fee once per year and co-pays when you receive service.Some families pay nothing; others pay up to $35.Pregnant women have no co-pays.

Eligibility

To qualify, children must:

  • Be 18 years of age or under
  • Be a resident of Colorado
  • Be a U.S. Citizen, a legal permanent U.S. resident for at lease five years, an asylee, or a refugee
  • Not qualify for Medicaid
  • Not have other health insurance
  • The family must also meet the CHP+ income and resource limits.

Colorado PEAK at  www.colorado.gov/peak  is a free, easy to use online pre-screening tool that can help to determine if your child may be eligible to receive CHP+, as well as apply for benefits online or check the status of current CHP+ benefits.

Certain expenses, such as child or elder care, child support, health insurance and others, may be deducted from your income.


How Do I Apply For Benefits?

There are several options available to you for submitting an application to the State CHP+ Office.

You need to complete anapplication and provide some documentation:

  • Download and print the online application.
  • Call CHP+ Customer Service at (800) 359-1991 Monday-Friday, 8 a.m.- 6 p.m. to have an application sent to you in the mail.
  • Visit the CHP+ office at 518 17th St., Suite 400, Denver, CO 80202 to request an application.
  • Mail a completed application to CHP+, P.O. Box 929, Denver, CO 80201.
